After review of video, it was determined that loose-fill surfacing material or unitary surfacing material under slide was lacking. Artificial turf does not meet acceptable standard of loose-fill material.
Four employees were missing training in annual content material.
A list of current employees was not posted. This was corrected at inspection when provider created the list and posted in the front foyer.
During walk through of the center I observed an IPAD in the infant room the IPAD was hanging and was turned on to a cartoon and children sitting on the floor were watching the cartoon. This was corrected when the director removed the IPAD from the room.
