Video footage of a caregiver smushing a child with a open hand on the forehead resulting to the child falling back and hitting head on a bookshelf was observed during inspection.

One caregiver was observed on numerous of occasions utilizing force towards a child in care. Based on the video footage observed, the caregiver was observed pushing the child into a seat. During the second incident observed, the caregiver was observed thrusting the child upon a chair.
Based on the video footage observed, one caregiver did not portray good judgment by picking up a child, whom is approximately 20 months of age by one arm. The child could be observed with their feet off the ground.
One child's epi-pen observed was not in the original container. The epi-pen did not contain the child's personal information, including the child's name.
