While reviewing video footage for an investigation, it was found that one caregiver was briefly over ratio by two children. This was corrected a few minutes later when another caregiver entered the room.

During an investigation it was found that infant feeding instructions could not be located for the previous month. I spoke with the director about keeping the previous infant feeding instructions.

It was observed on camera that a caregiver did not allow an infant to form his/her own pattern of sleep by continuing to lay the infants head down after the infant has woken. This was corrected, as the caregiver is no longer employed at the operation.

A caregiver gave a child a food item that was identified on the childs food allergy form. I spoke with the operation staff about trainings for allergies and other tools that can be used to prevent a reoccurance of this incident.

The employee listed posted did not list all current employees. This was corrected at inspection when the director updated and posted a current employee list.
