The operation only completed 3 of the 4 severe weather drills and 2 of the 4 lock down drills in the past year. This was corrected at inspection when a severe weather and a lock down drill was completed during the inspection.

The annual sanitation inspection was over due and last completed on 12/6/2023.

Out of 8 staff files reviewed, 2 staff did not have current first aid and cpr training.

The operation has not conducted a fire drill since December 2023. This was corrected at inspection when a fire drill was conducted.

The operation did not have their most recent inspection form posted in a prominent place where parents can see it. This was corrected at inspection when the inspection form was posted on the bulletin board.

It was found during an investigation that a caregiver was not providing adequate supervision when napping children were being monitored via video camera and did not have a caregiver in the room.

It was found during a self-reported investigation that a caregiver used prohibited punishment when the caregiver hit a child on their back. This was corrected when the caregiver was immediately terminated.

Out of 4 pesonnel records reviewed, one record did not have the 24 hours of annual training.

The director did not have current training in CPR and First Aid. The certificate expired on 2/24/22.
