Out of 4 staff files reviewed two were missing documentation on how they meet the educational qualifications.

Documentation was not available to show completion of shleter/lockdown drills, and monthly checks of the smoke detector, fire extinguisher and carbon monoxide alarm..

Two epi pens were observed to be without their original boxes.

Out of four staff files reviewed two were missing the required annual training.

Out of four staff files reviewed three were missing notarized affidavits, two were missing employment begin dates, one was missing a copy of their photo ID, and two were missing orientation.

Food allergies were not posted at the operation.
