An annual sanitation/health inspection has not been conducted and has expired.

During the inspection 3 of the 4 staff files were missing annual trainings in Food allergy, Administering Medication, Indfectious Disease, and Haardous Material trainings.

During the inspection the center director did not have the required annual trainings.

It was determined that good judgment was not utilized when a rodent trap was placed in a licensed space and was engaged while children were in the area. It was also concluded that good judgment was not exercised when parents were not informed of the potential interaction between their children and rodent feces.

Loose fill around the composite play structure on the Pre-K playground was less than six inches.

Two staff members did not have current CPR certificates.

The last fire inspection was conducted on 10/17/2023

One of ten children's files audited did not have a special needs statement.

Five of ten children's files audited did not have a current healthcare provider statement.

One of ten files audited did not have current immunizations.

Three of seven staff files did not have signed affidavits.

Two trash cans were observed containing garbage without a lid.

One garbage can was uncovered in the second toddler class. This was corrected at inspection.
