The operation staff failed to ensure that all children were accounted for while transitioning them back into the building resulting in two children getting out the playground due to the gate being left open and being found over 200 feet away from the center close to a busy street. The two children were unsupervised for approximately 2 minutes before being found and taken back to the operation. The operation terminated the caregivers responsible for the incident as soon as it was discovered.
