A tcaregiver was observed taking on the phone, taking a personal phone call. The caregiver did not hear me entering the room.
A swing observed on the playground had a dent in the middle from a tree falling on it. Paint was flaking and causing debris. The rubber coating around the chain was harden and falling off.
The television in a toddler room was on a bookshelf and not anchored or mounted.
The outside of the facility was not in good repair and had walling outside by the playground that had cracks and holes in it.
