During review of record keeping 1 out of 10 files evaluated did not include all required infromation for the emergency contacts. Address for the emergency contact was missing.

Two staff are missing trainings under the following topics: Preventing and controlling the spread of communicable diseases. Administering medication. Understanding building and physical premises safety.

During review of Safety Practices the operation's first aid kit was found incomplete. Missing Items 1. Anteseptic Wipes or solution 2. Adhesive Tape

Druing review of personnel files 3 out of 3 files evaluated did not include original training certifcates for annual training received.

During review of records 2 out 6 evaluated did not have Physician information as requred.

During the inspection I observed a bottle filled with blue solution with no label of what was in the bottle. This was corrected when the director labeled the bottle soap and water.

During review of records 3 out of 6 evaluated did not have an address for the emergency contact as required.
