The CPR/FA for the Site Director and another caregiver were observed expired during the inspection.

The Director Certificate for the operation was not available for review during the inspection at the operation.

The personnel records for the Site Director was not available for review during the inspection at the operation.

Proof of education qualification for the Site Director and the staff present at the operation were not available for review during the inspection.

The children records observed had not been updated to the required 2935 Form. The forms utilized does not address swimming, limitations and restrictions, and/or development needs of the child.

I did not observe documentation of first aid for one of the caregivers who was observed working with children during the inspection.

I evaluated 5 personnel records. I observed the following: 1. Two personnel files did not have a documented date of employment. 2. One file did not have documentation of that the age and education requirement had been met. 3. One file did not have a notarized affidavit 4. One file did not have documentation of orientation. 5. One file did not have documentation of abuse/neglect training. 6. The orientation documentation in 4 personnel files had not been signed by the trainer.
