During a records audit, it was found that all emergency contacts listed for the children were missing an address.
Upon inspection of the infant room, I observed there to be dirty bibs, utensils, and bottles inside of the hand washing sink. This was the only sink in the infant room.
It was found during the inspection that an employee had access to children without eligibility determination.
The vehicle used to transport children, did not have an electronic child safety alarm installed.
The operation space allows for 10 infants and at the time of the inspection there were 12.
