The fire extinguisher inside of the vehicle was not approved by a state or fire marshal.

The transportation van did not have the required safety alarm.

The epi pen was out of date.

The stairs plastic covering on the first after school playground was lifted and had exposed rough/sharp edges around the steps area. Eight pedals were broken on a five bike merry-go-round.

The two after school playgrounds lacked the proper nine inches of loose-fill surfacing around the play structure.

The form 7263 emergency practices, the fire extinguisher checks were not documented for the months January - September 2025.

Four out of the five staff records evaluated were missing the required training topics: emergency preparedness, preventing and controlling the spread of communicable diseases, administering medication, prevent and responding to emergencies due to food or an allergic reaction, understanding building and physical premises safety, handling, storing, and disposing of hazardous materials.

A review of the Employees Who Left Employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

There was not a safety mechanisim on the above ground changing table/area.

A current Employee List was not posted.

On the playscape there are protruding, rusty and sharp edges that are a safety hazard.

Inspector observed one person present at the operation who was allowed unsupervised access to children and did not have an initial background check or FBI fingerprint submitted and on file.

Three out of four staff members were missing trainings in section (e) of this standard. One staff member was missing training in food allergies and was missing 1 out of 24 hours. One staff was missing training in food allergies, building and premises safety, hazardous materials, and emergency preparedness. One staff was missing training in SIDS, shaken baby, and early brain development, communicable diseases including immunizations, administering medication, food allergies, building and premises safety, hazardous materials including blood borne pathogens, and emergency preparedness. This staff member had 7 out of 24 hours on file.

Inspector observed one caregiver step out of the room to get a stroller for the infants to go outside leaving the infants unsupervised.
