Based on the video and interviews conducted, a name to face was not conducted at each threshold nor was a sweep of the classroom before leaving out of the room. This resulted in a child being left in the classroom unsupervised for approximately 8 minutes while the class went outside.

Based on the video, a caregiver left the room during the beginning of naptime leaving one caregiver with 15 children, many of them were not on their mats asleep. A caregiver was in the bathroom assisting children and could not see the other children who were still playing around the room until she was finished and able to assist them with naptime.
