In the preschool classrooms, the children were not sleeping on their assigned cots. *Corrected during the inspection when the nap map was updated with the correct name and number for each child.
During the course of the investigation, it was determined that a caregiver did not demonstrate good judgement when she placed two infant bottles right next to each other just prior to feeding, then did not take the time to accurately identify the bottles before feeding them to the infants, leading to her feeding the incorrect bottle to an infant.
Caregiver provided the incorrect number of children in their care. Caregiver reported she had eight children in the classroom when there were seven children in the classroom. Note: This was corrected at inspection when the director had the caregiver verify the number of children in care. Caregiver confirmed there were six children in the classroom.
Twenty four school age children ages five to six years old were in the classroom with one caregiver. The specified age group was five years old, resulting in the classroom being over ratio by two children. Note: This was corrected at inspection when the director stayed in the classroom until a second caregiver was placed in the classroom.
The caregiver in the after school classroom gave the incorrect number of children in care in the classroom. Caregiver stated there were twenty-six children in care when there were twenty-five children in the classroom. Note: This was corrected at inspection when caregiver recounted the children and provided the correct number of children in their care.
During an investigation, it was determined a child exited their classroom when parent opened the door to the classroom. Child was unsupervised for approximately forty-five seconds until found.
Based on information gathered, caregiver was not actively supervising the children appropriately. Caregiver did not intervene when one child was on top of small bookshelf, and another child pushed the child who was on top of the bookshelf, causing child to fall over the bookshelf and the bookshelf flipping over and falling on child, causing a bruise near the eye.
The caregiver did not demonstrate good judgment when seen on video footage lifting a child by the left hand/wrist off the floor before getting a hold of the child’s right hand/wrist, extending both arms, and placing the child in the chair, which resulted in an accidental injury that required medical attention.
A staff member who had direct access to children in care did not have a background check from the CBCU regarding the persons eligibility. Note: This was corrected at inspection when caregiver was sent home.

Based on video footage obtained, a caregiver was observed forcibly approaching a young child and positioning herself over the child lying on a mat while grabbing the child's face in an attempt to address her behavior during rest time.
The parent whose child was left unsupervised on the playground was not notified immediately after the incident occurred. The incident occurred on October 11, 2024, and the parent was not notified about the incident until October 23, 2024.
The center director failed to meet her responsibilities and did not inform the parent of a child who had been placed at risk even after she had been notified that an investigation was opened regarding the incident.
Supervision was compromised when caregivers left a small child unsupervised on the playground. The caregivers who were on a separate playground found the child approximately 15- 30 minutes later and alerted management.
An incident report for an incident that placed a child at risk was not provided to the parent within 48 hours.
It was determined in a DFPS investigation that an incident that occurred on 10/11/2024 where a child was left unsupervised on a playground was not reported in the required 48 hour timeframe.
Documentation of a fire inspection being completed within the last 12 months was not available for review.
A shelf in the infant classroom was observed to have peeling safety coating. A large hole about 5-12 inches wide and 5-12 inches deep was observed in the school age playground. Exposed piping was observed in the school age playground.
