Loose fill on the playground was found to be under the required height of six inches. Some areas had up to 4 inches of loose fill. Other areas had only up to 2 inches of loose fill in the required use zone.

Two caregivers are past due on renewing their CPR training which expired in in January 2025.

Based on the evidence obtained during the investigation, an individual has been volunteering with unsupervised access to children in care without an eligible background check notification.

A caregiver was observed conducting diaper changes and did not wash their hands after changing diapers or assisting children going potty.

A caregiver stepped away from the diaper changing table while a child remained on the diaper changing table on two different occassions during the same diaper changing session. The staff member turned their back completely and stepped away to throw away a diaper and then a second time the caregiver left the diaper changing area and entered the bathroom to assist a different child going potty while the other child remained on the diaper changing table.

Based on the evidence obtained during the investigation, children are not washing their hands after toileting.
