During nap time it was observed on the video that a caregiver was not adequately supervising children. The caregiver was observed to be preoccupied doing something else and not supervising children.
Based on the information obtained from the investigation, it was confirmed that a child was left unattended on the playground while in care at the operation.

An employee with a hire date of November 15, 2021 was present in a room with children but did not have an eligible background check result allowing them to be present. The employee left the operation at the time of inspection and will not return until an eligible background check result is received.
