It was determined throughout the course of the HHS Investigation the caregiver failed to maintain the level of supervision required for a young child in care while on the playground. Surveillance footage revealed the child was out of the caregiver’s view for a total period of 6 minutes. Within those 6 minutes, the child was left alone outside unsupervised on the enclosed playground for 3 minutes and 15 seconds.
