Documentation showing the ages of children in care was not available upon request during the inspection.

Children in the toddler room were observed watching YouTube.

During the inspection a caregiver left a classroom of children unsupervised.

The incident report did not have the required information, it is missing directors name, operation number, if childcare regulation was notified, describe of the injury, if first aid provided, EMS was called and was the child transported for medical care.

The operation did not have loose fill under the moving merry go cycle.

During the walk through a caregiver was seen outside and not in close proximity of the classroom, leaving 9 children in care unsupervised. I also observed children on their phones.

Seven operational staff were review. One out of seven operational staff do not have required annual training clock hours.

Four children were observed playing video game name "Street Fighter" in a NES Nintendo System. The video game is seen doing street fighting violence. Note: It was correct during the inspection.
