Two slide equipments in the playground areas were observed without surfacing in their use zones. The height of the highest designated play surface on the equipments were over 20 inches tall. NOTE: This was corrected during the inspection by Director of operation. Both slide equipments were removed from the operation.
There was medication that was not being administered per the label instructions. *This was corrected at inspection when the medication was removed and sent home.
