I observed a school-age child placing their hands in the garbage can and rolling the garbage can with wheel around the cafeteria area. Gargage accessible to children and rolling the gargae can around pose a risk to the health and safety of children. The child stopped placing their hands inside of the trash can and stopped rolling the trash can around in the cafeteria when management intervene.

It was determined during the investigation that a caregiver did not demonstrate competency. The caregiver knew there was rule in place by the YMCA to not allow children to use the monkey bars; however, the caregiver allowed multiple children to use the monkey bars without stopping them, and it led to an injury to a child later in the day.
