Based on the evidence obtained it is determined that the operation was not in compliance with supervision as the caregiver was not able to intervene to avoid the accident form occurring.
Outdoor activity plan only has one opportunity for outdoor play. A minimum of two outdoor opportunities needs to be included in the activity plan.
Four nail head protrusions within children's reach were observed in the outdoor play area of the operation. Exposed nails were located next to the main playground area. NOTE: This was corrected during inspection. Protrusions were removed by maintenance staff.
