Evidence gathered during this investigation shows a caregiver did not have water safety training prior to taking a group of children swimming. The caregiver took the training after returning from the swimming field trip.

Children's records were reviewed during the inspection. Several children's records were missing health statements, immunization records, and parent rights information.

Monthly fire drills are not being conducted. The last fire drills was conducted in January 2026.

During the course of the DFPS investigation it was determined that day care staff yelled at a child in care.

During the course of the DFPS investigation it was determined that a child was not properly supervised when the child went back into the classroom while the rest of the class was still on the playground.

During the unannounced inspection, children did not wash their hands after going to the bathroom.
