The caregiver turned away from a child that required supervision during meals. This resulted in a lack of supervision as the child was able to reach for a cup of milk not assigned to them. The child was able to place the cup of milk against their lips and it is alleged child consume the milk due to the child’s severe allergic reaction symptoms.

Based on information gathered during investigation, staff failed to attend a child with a severe allergic reaction to milk. Staff was aware child placed a cup of milk to their lips and only removed the object, but did not rinse the child’s mouth or monitored the child for symptoms. Staff neglected to use good judgement towards the evaluation of the child and lack of communication with staff.
