There were seven expired medications in the stored medication for children. Note: During the inspection they were removed from the stored medications to return to the parents.

Documentation of current annual training for SIDS/Shaken baby was not available for two caregivers required to have it.

In the Cubs II classroom both children were not buckled into their chair while sitting at the table. This was corrected during the inspection.

Based on the information gathered a teacher did not show good judgement when she yanked up on a child's arm causing his elbow to be dislocated and causing his arm to be fractured per video.
