There was sufficient evidence and statements to support the allegation that diaper changing of children in care were conducted on non-changing equipment.
During investigation inspection broken wood pieces, nail pieces from the broken/ unrepaired gate and trash on the playgrounds were observed.
Play equipment was observed broken and damaged.
The employee list was not validated by Jan 15. Nor the extended 10 days to validate which was by Jan 25.

One caregiver observed in the afterschool classroom with the Toddler age group was observed utilizing her cell phone. The caregiver was not aware that she was in charge of the group during naptime. The caregiver was informed immediately by the Director that she was responsible for the age group, and that cell phone use is prohibited.
One out of four staff records observed did not have the training certificates available for review. The Director stated that the training certificates have not been placed in the staff files. The training certificates were not at the operation available for review.
