An infant was observed sleeping in a restrictive device. This was corrected at inspection when the child was removed and placed directly in the crib.

The rubber mulch in the use zone of the todder playground was less than six inches. The pea gravel in the use zone of the Pre-K playground was less than six inches.

A spray bottle in a classroom containing a bleach and water solution was not labeled and a bottle containing water and air freshener solution in the same classroom was mislabeled as bleach and water. This was corrected at inspection with the bottles were properly labeled.

One of eight staff files audited did not have proof of educational requirements.

A caregiver with a status of pending was with children. This was corrected at inspection when the caregiver was removed from the classroom.

One classroom did not have a current evacuation route posted. This was brought into compliance when the person in charge printed and posted an evacuation route.
