Based on the information obtained throughout the course of the investigation, it was determined that operation failed to timely report to CCR an incident that took place at operation which required a child to receive medical treatment.
High school diploma / GED or equivalent was not available for review for one out of six staff files reviewed. **Two files are pending verification regarding the validity of the high school diploma.
A toilet located in the second floor afterschool room was missing the toilet seat.
The First Aid Kit in one of the transportation vehicle was incomplete. The following items were missing: 1. Tweezers, 2. A guide to first aid and emergency care. Note: This was CORRECTED during the inspection as the missing items were obtained and included in the first aid kit.
