During the investigation staff left a child in the hallway unsupervised for at least six minutes.

During the investigation the operation self reported a child was left unsupervised during transition.

Based on the information obtained, it was determined caregiver did not account for all children in care during transition time when they left the room to go to the office.

Based on the information obtained, it was determined caregiver did not provide adequate supervision when child in care was left in the classroom unsupervised for five minutes.

One caregiver is missing one hour of child maltreatment training.

The operation had not submitted the total number of employees who ceased working at the operation the previous calendar year by January 15, 2024.

There was a parent volunteer with no active background check alone with 3 children. Note: Admin Penalty is attached. Note: Volunteer left the operation.

I observe several pieces of wooden logs deteriorating and three large nails protruding along the fence in the playground area.

Based on the information obtained, it was determined caregivers did not count the number of children after returning from playground.
