A caregiver left her class of eight children with a parent while she went to get another child from another classroom.

A child was left unattended on the playground.

The operations fire inspection expired 4/1/2022.

The staff did not follow the operation's policy for arranging a parent meeting to talk with parents. Instead 2 staff engaged in a conversation with a parent which took away from their ability to supervise children adequately

The operation completed an outdoor check but did not address all hazards. The operation did not close an open gate on the infant/toddler playground which resulted in a child being able to leave the operation premises.

A child was able to get out of the fence and wonder away from the operation. The child was away for approximately 9 minutes and the staff did not know where the child was. The child was returned by an unknown person.

The operation allowed a caregiver administer medication without medication training.

Four of ten staff records reiewed did not have 24 hours of annual training.

First Aid Kit incomplete missing tweezers, scissors, adhesive tape, a guide to first aid and thermometer.

Four caregivers were present in classrooms either by themselves or with an additional staff member without eligible background checks. The classrooms ranged from infants, toddlers and Pre-K.

I observed a 3 month old in his crib swaddled.

The operation didn't notify licensing within 48 hours of being made aware of a situation placing a child at risk.

A two year old child was left outside on the playground for five minutes unsupervised.
