A parent was not informed of his child's minor injury at pick-up.

A caregiver did not intervene appropriately to prevent an infant from falling off an 18-inch platform on the playground and suffering a minor head injury (e.g., half-dollar sized circular bruise on forehead). The caregiver was terminated as a result of the accident.

It was found during the inspection that a child tested positive for COVID and it was not reported to Licensing or to all parents and staff if the operation.
