1/There were 2 missing wheels, broken wooden bottom of the two red cars using by children for outdoor activities. 2/There were some broken floor tiles in baby room and on the hall way to children restroom posing tripping hazards. Note: Pictures taken and showed the director during this visit.
The operational policy was missing the written plan for the promotion of physical play.
There were 4 out of 4 staff records that did not have the pre-employment affidavit avaiable for viewing.
There were broken fence boards in the outdoor play area accessible to children in care.
A caregiver was allowed to work in the infant room without the eligibility determination. Note: The caregiver was sent home and will not be allowed back until all background checks are cleared.
In the infant room there was one broken tile and a baseboard that falling apart.
The outdoor play structure had broken parts throughout the structure exposing sharp edges.
