The preschool and school age playground lacked the required 6inches of loose fill surfacing.

The large playground was observed to have animal feces, broken glass, exposed wires, broken toys, trash, and fire ants. A protruding barrier stake was also observed around the playground structure.

Based on investigation by DFPS, caregivers were observed on their phones while supervising children.

The following trainnig is not current for an employee: Recognizing and preventing shaken baby syndrome and abusive head trauma; Understanding and using safe sleep practices and preventing sudden infant death syndrome(SIDS); Understanding early childhood brain develpment and Child Maltreatment.

A review of the Employees Who Left Employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Based on the information gathered, a child was not supervised by a caregiver in the playground due to caregiver completing other work duties. This allowed the child to squeeze through the bars of the gate causing her to wander outside the operation unsupervised for approximately 10 minutes. Child was unharmed.
