This standard was tasked as part of the investigation and found to be deficient. A child who is a known runner was able to run toward and fall through the fence on the playground and escape.

This deficiency was tasked as part of an investigation and found to be deficient. The fence on the playground had damages which caused a child to fall through and escape without injuries.

This standard was tasked over for this investigation conducted by DFPS and found to be deficient. Following this investigation, it was determined that caregiver(s) did not notify that parents of the accident and/or injury in a timely manner. Parents were not notified of the child's discomfort until the afternoon, and the child was showing signs of discomfort throughout the day.

This standard was evaluated as part of an investigation and was found to be deficient. The caregiver did not have auditory or visual awareness of 2 children when they were allowed to return into the building alone.

None of the cots or cribs were labeled.

The amount of time the children sit in time out at the operation is not consistent with the ages of the children in care.

The operation recieved a minor citation on their sanitation inspection conducted on 9/28/2022 for kitchen staff not having food handlers certification. The kitchen staff still has not completed a food handers certification course.

5 caregivers had affidavits that were not notarized.

Two children were observed sleeping in restrictive devices. This was corrected when the children were removed from the restrictive device and placed in the crib.

Two children were sitting in restrictive devices while inside of the crib, causing the crib to not be bare of anything other than the child. This was corrected when the caregivers removed the restrictive devices from inside of the crib.
