During the investigation, it was determined that an individual who had received an ineligible background check was present at the operation.
During the investigation it was found that the children did not demonstrate an understanding of the sticker reward system. As such, the guidance method in use does not align with the developmental level or comprehension abilities of the children in care, rendering an ineffective disciplinary action.
Based off evidence gathered, black substance that appeared to be mold was observed on the ceiling of a restroom used by children. The area had not yet been officially tested or treated for mold as of August 2024.
Based on the information gathered the center staff failed to report the communicable illnesses within the required time frame.
