During the walk through of the operation it was found that in one of the infant rooms with the Specified Age Group of 0-11 month olds there was one caregiver and five infants therefore this room was over the required ratio. This was corrected during the inspection as the 2nd caregiver came back into the room after her break.
1 out of 10 records reviewed found that one caregiver did not have the hands on skill based CPR training required.
Evidence obtained during the Abuse/Neglect investigation found that caregivers were not supervising the children appropriately. As of result of the caregivers not being in close proximity and intervening in time a child received 2nd degree burns to the hands.
It was determined during the DFPS investigation that a 6 year old child was left unattended on the bus.
It was found during the inspection the operation allowed a person to be present at the operation and have access to the children before receiving notice from the Criminal Background Check Unit regarding the person's eligibility.
