During the inspection 2 children left the classroom to go to another classroom across the hall. Both children were unattended.

The operation is providing care for an infant when they are licensed for toddler-school age care.
Clorox clean-up wipes were observed being stored on a small shelf that children could easily access. Note: This was CORRECTED AT INSPECTION when the caregiver moved the Clorox wipes to a high shelf that children cannot reach.
The slide on the pre-k playground did not have 6 inches of loose fill. Loose-fill was measuring at about 5 inches deep.
The grass on the pre-k playground was observed to be overgrown measuring at approximately 7in high.

The evacuation diagram was missing the designated location inside the operation for children and staff to meet to take shelter from threatening weather.
The fence surrounding the outdoor play area had a broken board and gaps between the fencing.
