Two children with known food allergies do not have a food allergy emergency plan available for review.

In a Pre-K classroom, there was one caregiver to 16 children in care. The standard age was 3 years old. This put the child to caregiver ratio out of compliance by having one child to many.

During a DFPS investigation, it was determined that a caregiver grabbed and pulled a child by the arm on at least three separate occasions throughout the same day. In three different incidents, the caregiver is observed to grab the child by their wrist, forearm and later by the upper arm. In another situation, the class is entering the operation from being outside and the same caregiver is observed carrying the same child and once inside, the caregiver drops the child a few inches off the floor and the child begins to cry.

This standard was found deficient as part of a DFPS Investigation.

Two out of ten employee files checked did not have a copy of the employee's identification available for review.

The operation's fire inspection has expired as of 12/16/21.
