Information gathered in the investigation substantiated that pita chips were served to an infant aged child who had a choking incident in care. The pita chips had not been broken up and were served whole. Information gathered substantiated that pita chips are a regular menu item being served to children 12 months and up.
During the walk through it was found that two classrooms did not have the mat chart updated with child's name and which nap cot. Note: This was corrected at inspection when the mat chart was updated with children's name. The nap cots are labeled by number.
