Caregiver did not wash their hands before serving children pizza, caregiver only place gloves on their hands.

Caregiver stated to children " if you are not going to listen you will not get a cup cake." Caregiver stated to a child, "child name you are not getting a cup cake and I going to tell your mom and TiTi."

Three children specified age group 3 years old were outside of the classroom with out adult supervision. Children were allowed to go to the rest alone, the rest room is located on the other side of the cafeteria from the classroom the caregiver is in.

One caregiver had 20 children specified age group 3 years old, the caregiver was out of ratio by five children. The other caregiver walked out of the room to grab something from the front office.

The operation is missing required information in the operational policies and did not have required information for this particular location. The operational handbook was a corporate policy. (Areas from 744.501 1,2,7,8,9,17, and 20).

The operation had two expired medication for a child that had a food allergy.

The fire marshal inspection report conducted on 1/7/25 had a deficiency that has not been corrected yet.

An annual sanitation inspection report was not available to review.
