The operation gave a two-year-old child Children's Benadryl and Hyland' s Kids Cold & Cough at the same time as authorized by parents. The medications were given in a syringe, which is against the label instructions, which state "Measure only with the dosing cup provided".
An incident occurred which resulted in two children's medications being swapped. The two children have the same first name.

One caregiver was observed using an iPad to take pictures of the children’s activity and the other caregiver was preparing the next activity taking the caregiver's attention away from the children. One child pushed another child causing an injury.
