Three transportation vehicles observed did not have a complete first-aid kit available. The first aid kit observed did not include antiseptic solution, a thermometer, tweezers, and a guide to first aid and emergency care. The transportation vehicles were utilized during AM transportation services. The operation Director was able to obtain additional supplies from the office in order to add to the transportation vehicles.

The transportation driver did not utilize the list of children to conduct a name to face check to ensure all children were dropped off to their designated location. Although, there was a list of children on the transportation vehicle, the driver did not follow the policy, which resulted in a child being dropped off at an incorrect location/school.

Based upon the information obtained during the investigation, sufficient evidence was available to support children under 3 years of age were allowed to play with Water Beads while engaging in a sensory activity.

Documentation was not available for review during inspection to verify a gas inspection had been conducted on/before 02/23/2024.

In the 3-year-old classroom, a caregiver's backpack purse was on the observed on the table assessible to children. The caregiver was informed of the deficiency. Corrected during the inspection when the caregiver removed their purse and placed it out of reach of the children.

Based upon the information obtained during the investigation, sufficient evidence was available to support, a child had not been properly supervised. A child was able to leave their classroom, walked down the hallway then out of the building without the caregiver(s) knowledge.

Based upon the information gathered during the investigation, the facility failed to report they discovered a child had razor blades in their possession and passed them out to other children.

The CPR training for a caregiver who assisted a child with an injury was obtained through NationalCPRFoundation which is online training.

Based upon the information obtained during the investigation, a parent was not notified at pickup their child sustained an injury while in care.

Based upon the information obtained during the investigation, sufficient evidence was available to support the operation failed to contact the parent of a child who became ill while in care prior to be transported to their elementary school.
