Based on the information obtained through the DFPS investigation, sufficient evidence was available to support a caregiver failed to demonstrate good judgment and self-control by inappropriately placing a child on the floor and to the table.

Based on the information obtained through the DFPS investigation, the facility failed to report an incident that placed a child at risk.

A caregiver failed to wash their hands and the hands of a child after diapering. Note: The director was informed of the deficiency during the inspection and stated she will be meeting with each caregiver individually and have them to review the handwashing policy as defined in the minimum standards.

A four-year-old child was not properly supervised by the director when the child left the classroom and walked out of the building.

It was observed a caregiver in the infant classroom with children and had a pending background. Corrected: This was corrected when the caregiver left for the day.

The incident report did not have the required information (operation name and number, name of the director, child's date of birth, time of incident, if parents and child care regulation was notified and parents signature). Corrected: When the director added the information on to the operation's incident form.
