During the inspection was it found that the operation did not obtain background check information for a staff member of soccer shot who was observed doing soccer practice alone with the children.

One out of ten child records reviewed did not have a completed immuization record.

1 food allergy plan was not signed by a health care professional.

4 out of 6 children's records were missing the address for the emergency contact. Note: This was corrected at the inspection.

2 out of 6 children's records were missing the admission date. Note: This was corrected at the inspection.

During the investigation inspection I observed a new employee supervising a group of children alone during naptime.

Based on the information obtained, it is determined that the three hired caregivers did not have a cleared eligibility check before having access to the children in care.

Based on the information obtained, it is determined that one out of three caregivers did not have pre-service training before being counted in the child/caregiver ratio.
