The fire drill for the month of May 2026 was not completed.

A minimum of 25 hours of business management training has not been completed for each month and training certificates were not available for review at the time of inspection.

Playground maintenance checklist was last completed on 8/2024.

Individual storage space was not labeled with children's name.

The loose fill around the climbing structure is 3 inches not the 6 inches required.

The transportation binders were missing medical authorization for some of the children being transported.

The fire extinguisher was unsecured in all 4 transportation vehicles.

Four children's records reviewed were missing complete emergency contact information.

The thermometers in the first aid kit were not operational due to the batteries being expired. This was corrected at inspection when an operational thermometer was added to the first aid kit.

The fire extinguisher in the van was in red on the safety gauge which renders it unsafe and unreliable for operation. This was corrected at inspection when an operational fire extinguisher was provided and put in the van.

Director's first aid and CPR expired on 12/17/2021.

The gas inspection expired on 1/22/2020.
