Children under two years of age were watching TV while getting ready for nap.

The person in charge did not have access to all personnel records that were requested by CCR.

During the inspection, 5 individuals were observed to be present and have access to the children at the operation before receiving notice from the CBCU regarding the person's eligibility. This was corrected during the inspection when all 5 individuals left the premises.

During the inspection it was observed that the operation's emergency prepardness plan does not have the name and address of the alternate shelter used for evacuations. This was corrected during the inspecction when the emeregency prepardness plan was updated with the required information.

1 out of 10 children’s immunization records reviewed did not have the correct form provided by the Department of State Health Services reflecting exemption.

It was found during the inspection the operations last fire inspection was conducted on 3/28/2022.

While reviewing the Emergency Drill Sheet it was found that the operation did not conduct a fire drill in February or June of 2022 and in June of 2023.

It was observed during the inspection after a child's diaper was changed the caregiver did not wash the child's hands under running water and the child had control of their neck muscles and was able to reach for the water. This was corrected during the inspection as the caregiver washed the child's hands after it was brought to their attention by Licensing.

It was observed during the inspection that a caregiver used the 4 step sanitizing process after a diaper change but wiped the bleach solution after spraying instead of allowing the solution to air dry. This was corrected when Licensing brought this to the caregivers attention.

It was observed during the walk through of the operation a caregiver was watching the TV Series Suits on their personal cell phone while the children were napping.

2 out of 2 Director files reviewed were missing the Director certificate and were not available for review during the inspection.

1 out 6 records reviewed showed a caregiver was missing 13 Pre-Service training hours required and has been employed longer than 90 days.

5 out of 6 records reviewed did not have documentation of completeing Pediatric First Aid or Pediatric CPR.

1 out 6 records reviewed did not have verification the employee completed orientation.

1 out of 6 records reviewed shows that Pediatric CPR expired on 5/2022.

1 out of 6 records reviewed shows one director had verification of completed 2 hours out of 30 hours required for training year 5/2021-4/2022.

Evidence obtained during the investigation found that a caregiver did not use good judgement which resulted in a child being injured.
