Life Time Fitness Camps

A child was observed asleep in the operation staff office without a staff member present.

During the follow up inspection there were three caregivers present, interacting and responsible for their own group of children that did not have an active background check. All three caregivers background checks have been submitted and are scheduled for fingerprinting. This was corrected when the director removed the caregiver from ratio.

During the follow up inspection it was found that the director allowed three staff members to care for children without an active background check. This was corrected when the director had the caregivers go home for the remainder of their shifts.

During the inspection there were 11 caregivers present and 5 caregivers did not have a background check. This was corrected when the director had the staff members without backgrounds leave and ran their background checks in the provider portal.

During the inspection it was found that the operation had three first aid kits, however each of them were missing an item or two. This was corrected at inspection when the first aid kit for the fitness camp was made complete.

The last annual health inspection report is dated 1/20/23. A current report was not available.

In the family changing room shared space, there are hairspary and other chemicals on each sink counter.

There was no documentation for a fire drill completed for the month of June.

During the unannounced inspection, there was no director present.

There were no current training hours for the required training needed in the director's personnel file.

There was no current CPR or First Aid training certification available for review.

An electrical out was uncovered in a space occupied with children. This was corrected during the inspection.

There was no documentation that annual training was complete by any caregiver.

It was found that the drills were not documented.

One out of eight caregiver training reviewed is missing the training length on the certificate.

There were three caregivers counted in ratio without a valid background check. Note: They left the operation

The CPR/First aid training, the Pre-service training and educational qualifications was not available able for review.
