Evidence obtained during the investigation shows that an employee of the operation did not use good judgement or self-control when they held on to and pulled a child's arm in an attempt to remove the child from the parents' arms while the parent was attempting to leave with the child.

It was found during the investigation that caregivers left children alone inside of a classroom while the children were napping.

It was found during the investigation that staff inappropriately disciplined children by using their hands to hit children.

During the inspection it was observed that the last fire inspection was conducted on 3/22/2021.

A monthly ground checklist was not available for review during the inspection. Corrected during inspection as director completed the form dated 11/28/2022.

2 out of 6 immunization records reviewed did not have verification of being up to date or have exemption or exception at admission and current shots.
