A school age child was observed watching YouTube videos on his smart phone inside the school age classroom.

A total of one caregiver and seven children were inside the infant classroom. The specific age group for the classroom is twelve months to seventeen months old. The infant classroom is out of ratio by two children.

The operation did not have the latest 2936 inspection form posted for licensing to review.

The operation did not have the daily menu for all meals and snack served inside the childcare center for licensing to review.

A total of three operational staff were review. One of the three operational staff did not have their operational staff file complete for licensing to review.

The playground slide was observed and did not meet the six feet use zone for children to exit successfully out the slide into the playground. Note: The standard was corrected during the inspection.

A total of three operational staff were review. Two of the three operaitonal staff did not have their high school diploma for licensing to review.

A walkthrough was conduct inside and outside the operation. Rodent bait stations were observed outside the school age playground and it accessible to the children. Note: The rodent bait stations were removed from the playground and correct during the inspection.

The three playgrounds are not properly set up yet. The operation is still missing critical equipment and children structure for outdoor activity use.
