Four infants were placed in their cribs with sleep sacks that restricted the infants arm movement. The sleep sacks have additional clothes on the side that fold over the infants arms.

The health inspection form from the country was not available for review.

Two outdoor playground equipment lacked the required 6 inches of loose fill surfacing under and around the equipment

During inspection, a caregiver who did not have an active background check was present in a classroom. Note: This was corrected at inspection when the caregiver was removed from the classroom.

The required Consumer Product Safety Commission form has not been completed within the last year. Note: This was corrected at inspection when operation signed up and printed and posted signed form.

The transportation vehicle's fire extinguisher had an expired inspection. This was corrected during the inspection when it was replaced with a fire extinguisher that has a current inspection.

One prescription for one child was not in the original container. This was corrected during inspection when the parent sent the prescription label for the medication.

Four medications were expired at the time of inspection. This was corrected when the medications were discarded.
