A child was found unsupervised outside of the classroom.

Based on the information gathered during the course of the investigation, the operation was not following the parent infant feeding schedule for each child and was using their own schedule in which to feed the babies.

One infant not yet on table food did not have current infant feeding schedules. The last infant feeding schedule was August 2023.

The operation did not have a current employee list posted at the operation.

The emergency preparedness plan did not address how children with limited mobility or who are under 24 months will be evacuated or provide an address for an evacuation.

At the time of inspection the operation did not have a fence for the outdoor play area.

At the time of inspection the operation did not have a fire inspection completed.

At the time of the application inspection the provider did not have an environmental inspection or a letter to verify that an inspection is not needed.

During the inspection, the inspector observed that one room had wires hanging from the ceiling. Two other classrooms were dim and require more lighting.
